Lifeline Subscriber Agreement

Rules and responsibilities for Free Link Wireless Lifeline subscribers, including one-per-household, usage, and recertification.

This agreement describes the federal Lifeline rules that apply to your Free Link Wireless service.

1. Eligibility

You certify that you, a dependent, or a household member participates in a qualifying program or meets the income limit, and you will provide proof when asked.

2. One per household

Lifeline is limited to one discount per household. You certify that no one in your household receives another Lifeline benefit.

3. Non-transferable

Your Lifeline benefit may not be transferred, sold, or given to another person.

4. Usage requirement

If your service is free, you must use it at least once every 30 days by making a call, sending a text, using data, or responding to our contact. If not, we'll notify you and give you 15 days to cure before de-enrolling you.

5. Annual recertification

You must recertify your eligibility every year. Failure to recertify will result in de-enrollment.

6. Notify us of changes

You must tell us within 30 days if you no longer qualify or if someone in your household begins receiving Lifeline, and notify us of address changes.

7. Information sharing

You authorize us to share your information with USAC, the National Verifier, and state administrators to verify eligibility and prevent duplicate enrollments.

8. Penalties

Willfully making false statements to obtain Lifeline can be punished by fine or imprisonment or can result in being barred from the program.

9. Transferring your benefit

You may transfer your Lifeline benefit to another provider, subject to program rules.
